Hero slime

There was a young human who wanted to be a hero more than anything. But the evil wizard cast a spell on him, and he was reduced to a small, squishy slime monster.
Without his strength, he can't fight; without his knowledge, he can't seek help; and without his intelligence, he doesn't realize what he's become.
He doesn't even remember his own name.
But he still thinks he's a hero.
